HIPAA Notice of Privacy Practices

This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our commitment

Milos MD, together with the LocumTele provider network and Rush Pharmacy, is committed to protecting your Protected Health Information ("PHI") as required by the Health Insurance Portability and Accountability Act (HIPAA).

Uses and disclosures for treatment, payment, and operations

  • Treatment: Sharing your intake with the reviewing provider and with Rush Pharmacy so a prescription can be evaluated and dispensed.
  • Payment: Processing payment for consultations and medication with our payment processor.
  • Operations: Quality assurance, compliance, training, and other business functions carried out under appropriate confidentiality safeguards.

Other permitted or required disclosures

We may use or disclose PHI as required by law, for public health activities, to report suspected abuse or neglect, for health oversight, in judicial proceedings, to law enforcement in limited circumstances, and to avert a serious threat to health or safety.

Your rights

  • Request access to and a copy of your PHI.
  • Request amendment of PHI you believe is inaccurate or incomplete.
  • Request an accounting of certain disclosures.
  • Request restrictions on certain uses and disclosures.
  • Request confidential communications at an alternative address or by alternative means.
  • File a complaint if you believe your privacy rights have been violated, without retaliation.

We may change this notice at any time. Revised notices apply to PHI we already have and to PHI we receive in the future. The current version is always posted on this page.
